As of Monday, November 25, 2014, we are now located at 401 E. Congress Parkway in Crystal Lake, IL. We have a clinic now that is twice the size of our old clinic and we will be able to see twice as many patients as well! We are so fortunate to be able to offer this to our patients. Here
