What to bring to your first appointment

To help prepare you for your visit, we have provided some information below.

  • Appointments are scheduled ahead of time, there are no walk-in appointments available.  If you require immediate assistance, call 911.
  • Appointments can be made at 779.220.9300.
  •  Patients are charged on a sliding fee scale, however no one is turned away for inability to pay.

It is important that you bring necessary information with you to your first appointment.  The following is required: 

Proof of income – payroll stubs that cover your previous 4 weeks of income

A copy of last filed income tax return

2 months of bank statements

Picture I.D.

Proof of McHenry County residency, for example a utility bill, Driver’s license or State I.D.

If you were previously insured, and are not now, you need to bring a letter in from your employer stating you do not have health insurance through your work.
