To help prepare you for your visit, we have provided some information.
Appointments are scheduled ahead of time, there are no walk-in appointments available.  Appointments may be made at 779.220.9300.  Patients are usually seen within 3-4 weeks of calling for an appointment.  There is no urgent care available at the clinic.  If you require immediate medical assistance, call 911.

Patients should bring to their first appointment:

Pharmaceutical assistance may be available to Clinic patients.

Specialized programs include:

  • Breast Cancer – One of the areas that the Clinic has focused on is the access to early detection/screening for breast cancer.  With the support of the McHenry County Breast Cancer Task Force, FHPC developed a county wide program so that health care systems would rotate responsibility for low cost mammograms.  Additionally, the Clinic participates in the Illinois Breast and Cervical Cancer Program.
  • Celebremos La Vida – This nationally recognized program replicates a highly successful breast and cervical cancer screening program based in Washington, D.C.  Celebremos La Vida brings FHPC and Centegra Health System together in a program targeting uninsured Latino women who would otherwise go without these critically needed cancer screening services.  Once a month, the Clinic staff locates at the Women’s Health Department at Centegra Hospital in Woodstock.  Clinic staff conduct physicals, clinical breast exams and pap smears while the Centegra staff do the mammograms, all in an atmosphere of fun and festivities.
  • Patient Navigator – Funded by the McHenry County Mental Health Board, the Clinic has a Patient Navigator who can help patients find additional resources.  In addition, navigating a patient through medical services is offered.
